Before the consultation:

1. It is necessary to make an orthopantomogram (OPG) or a projectional radiography testing (RT) in side projection or computerized tomography (CT) scan, and make an appointment with an orthodontist. Requirements for CT scan

 

2. The orthodontist will create plaster models of the jaws, pictures of the face in five projections: full face, profile and semi-profile, intraoral.

With these materials you make an appointment with me for the consultation: in person, in the clinic or remote – by email.

 

If you need any further additional examinations or consultation of doctors of related specialties: orthopedist, implantologist, this will be known at the consultation.

 

3. We will make the treatment plan, discuss aesthetic changes of the face, plan our next appointment in order to control the progress of preparation for the surgery.

The treatment plan is generally standard and sometimes the protocol first takes place. As a rule, a standard three-stage approach (orthodontic treatment – surgery – orthodontics) is optimal. Non-disputable advantages – good predictability or results, complete rehabilitation from the orthodontic and surgical sides and a clear gradation of stages.

However, the Surgery First protocol is more preferable – two-stages treatment when braces are installed a few days or even hours before surgery, and orthodontic treatment is carried out after the movement of the jaws.

Preparation for a bite correction:

The first stage – the treatment with braces can be considered completed if you have a rectangular steel arch that fills the groove of the brace, and the form of the dental arches is such when the model of the jaws is matching multiple occlusion contacts.

In this case you make control plaster models of the jaws – casts, an orthopantomogram (OPG), a projectional radiography testing (RT) in side projection and computerized tomography (CT) scan. Upon completion you send me these materials for the conclusion and planning of the surgery.

1. I have informed you that you are ready for the surgery. What is next?

2. Two weeks before the surgery your tests for surgery should be at the clinic.

3. After the surgery you will be as an inpatient for one day. The next morning, we will have a review, bandage, and you will receive a referral for rehabilitation treatment – microcurrent therapy.

You will have to stay in St. Petersburg for 10 days after the surgery.

After the discharge I am observing the patient every 2-4 days, controlling the progress of rehabilitation. The rehabilitation procedures are prescribed individually.

 

They relieve pain, accelerate recovery, eliminate swelling and hematomas, improve microcirculation in damaged areas which helps to restore sensitivity faster. After the collection of anamnesis and examination, the procedures are carried out by a doctor cosmetologist with the appropriate qualification.

After orthognathic surgery

Fever in the first week, weakness and drowsiness after the surgery are possible. You should rest as your body is recovering. During the month you should avoid physical activities, tilts, bath and solarium procedures.

Hygiene

Rinsing of the mouth and nose is an important step. It is optimal to use herbal infusions with anti-inflammatory and decongestant: chamomile or sage. You have to rinse the nose and mouth every two hours during the day, in the morning after bedtime and in the evening before bedtime, after each meal.

Medicines

Diligently follow the doctor’s instructions and do not avoid prescriptions: antibiotics, anti-inflammatory and decongestant medicines are necessary for the quite course of rehabilitation, recovery of the body. The list of required medicines is individual and is given at discharge.

Sensitivity

For some time after the surgery numbness persists in the lips, jaws, chin, cheekbones. This is usual and connected with the surgical intervention. Sensitivity is restored from a few days to half a year after the surgery. In order to accelerate the recovery process, you have to do myogymnastics.

Bruises and swelling

To reduce swelling and hematomas you will have a bandage around the face for the first day after the surgery. Do not remove the bandage by yourself or without the doctor’s permission. Then swelling will increase – it is normal, the peak falls on the third day after the surgery.

 

Swelling can spread to the submandibular area, the neck and even the décolleté area. There is nothing wrong with this.

Both swelling and bruises will spread unevenly on both sides of the face and neck – it is regular. Rehabilitation procedures will accelerate recovery and convergence of edema.

Nose and nasopharynx

After the surgery, there may be a sore throat, nasal congestion, which is caused by swelling and passes in a few days. It is possible to have a release of blood clots from the nose for a few days after the surgery. Recommendation for the care of the nasal cavity is given at discharge.

Postoperative orthodontic treatment

If the proportions of the face are normalized, multiple occlusive contacts are reached, and the movement of the jaw fully restored – the surgical stage is completed, then we start the next one – orthodontics treatment.

 

The aim is the final alignment of the jaws, the creation of a normal facial aesthetics, a beautiful smile, a correct dental arch. After removing the fixing elastics in 3-5 weeks after the surgery, an orthodontist will begin the creation of a beautiful smile. If the treatment plan does not require additional manipulations: implantation, prosthetics, then after a short period of time the orthodontist will complete the treatment and start the removal of braces.

How will my face change?

When the upper jaw is put forward the flatness of the face is eliminated. The upper lip is moved forward, the nasolabial folds are smoothed. The tip of the nose rises. In case of movement of the upper jaw fragment upward the face is shortened, or rather its middle and lower third parts. Changes affect the nose as well: there is a slight expansion of the wings, and the tip of the nose is raised, the nasolabial angle is sharpened.

The putting back of the fragment of the upper jaw causes the elimination of the expressed position of the upper lip, drooping and rounding of the tip of the nose. With the orthodontic treatment the relaxation of masticatory (chewing) and mimetic (control of facial expression) muscles that affect facial features. Patients with bruxism (teeth grinding) face the problem of hypertonus of masticatory muscles. With the use of a mouth guard an orthodontist separates the dentition so the muscles can relax.

The second category of patients with a hypertonus problem – patients with temporomandibular joint dysfunction (TMD) dysfunction which have spasm of masticatory (chewing) muscles leading to chronic pain. In this case there is a goal to separate the teeth to relieve tension.
